LINK, Inc. provides independent living counseling services to individuals
ages 16-64, who are eligible for the Medicaid Physical Disabilities
Waiver.
Eligibility is determined by an assessment and a plan of care is written to meet the
individual's needs. Services can include Personal Services and Assistive Services
that help the individual maintain and live in their own home independently or with
assistance.
Individuals with traumatically aquired brain injuries may be eligible for the Medicaid
Traumatic Brain Injury Waiver. This program assists individuals in making
the transition to community living with a wide range of services. This is a more
intense program that works on skills that may have been lost or need to be refreshed in
the individuals mind. Working up to 5 hours a week, if needed, to make progress toward
the individual's goals. Setting and achieving goals is all part of this program and
achieving a maximum level of independence is the overall goal.
PAYROLL PROVIDER
LINK, Inc. acts as a Payroll Provider for Medicaid Home and Community Based Waivers which include : Physical Disability Waiver, Traumatic Brain Injury,
Frail-Elderly, and Mental Retardation/Developmental Disability Waiver.
When an individual chooses
to self-direct their personal care attendants, LINK provides a pool of applicants to choose from
or the individual may pick someone they know to assist them. To self-direct means to have the ability for you to direct someone how to best care for you, you are the
personal care attendants boss.
LINK has a staff member, Christine, dedicated to providing assistance in recruiting or advertising for personal care attendants.
